brauche Eure Mithilfe in diesem Fall, Patientin 66 Jahre:
Leitsymptomatik: Massive Splenomegalie, Panzytopenie, sehr hoher TK, Nachtschweiß, Müdigkeit-Schwäche,
KEIN Fieber,CMV + B.pertussis + Mycoplasmen + Chlamydien positiv.
Kein eindeutiger Knochenmarksbefund aus Beckenkamm Biopsie, weder CT noch MRT und Ultraschall Auffälligkeiten. KMB vor 3 Jahren ebenfalls ohne Befund.
Wachstum der Milz in 3 Jahren von 19cm auf knapp 30cm.
Im Kindesalter TB sichtbar auf Lunge, heute kein Nachweis, Lunge/Bronchen ohne Befund.
USA/Canaren/Tropenaufenthalte (Südost Asien) mit Mückenstichen, Sandfliegen.
Nun stellt sich die Frage: Splenektomie mit anschließender Biopsie der Milz. Milz weg, aber Ursache evt. nicht behoben. Brodelt dann evt. ein Infekt weiter, falls kein Lymphom in der Milz selbst gefunden wird?
Welche viralen, bakteriellen Infekte oder Immunerkrankungen kommen noch für eine derart seltene massive Vergrößerung der Milz in Frage?
Anbei die Labordiagnostik zum Tüfteln und wer möchte bekommt ein paar englisch sprachige Links, in Deutschland finde ich leider kaum Literatur zum Thema

Abstract
The tropical splenomegaly syndrome (TSS) is characterized by massive splenomegaly with hypersplenism, moderate hepatomegaly, and lymphocytic infiltration of the hepatic sinusoids. In previous reports this syndrome has been shown to be a consequence of a disordered immunologic response of the host to malarial infection. Treatment with antimalarial drugs has resulted in a decrease in malarial antibody titers and a reduction in splenic size. We report a child who had TSS associated with cytomegalovirus infection rather than malaria. Our results suggest that TSS may be precipitated by a variety of infections producing chronic antigenic stimulation and perhaps by autoantigenic stimulation as well.
